Introduction Hospital delirium is a frequent, serious, costly, and underrecognized acute disorder of attention and cognition. Therefore, the prevention of hospital delirium is not only desirable for patients, caregivers, healthcare professionals, and the health system itself, but also a moral duty. Objective To identify and synthetize the main ethical aspects that arise related to the prevention of hospital delirium in patients 65 years and older. Methods A scoping review was carried out in Embase, Medline, and Web of Science. Articles published (...) in English or Spanish analyzing any pharmacologic, non-pharmacologic, or multicomponent intervention for prevention of delirium in hospitalized patients aged 65 years and over were included. Results Twenty-seven studies were included. Based on the frequency of appearance and the relevance, our findings were organized into three main areas. The first one was related to the principle of nonmaleficence and collected findings about the iatrogenic causes of hospital delirium and the use of restraints systems. The second area analyzed the impact of delirium in patients, families, and healthcare professionals from a relational ethics perspective, and addressed the family presence as a factor to prevent delirium. Finally, the third area is related to the quality of care. Conclusion Prevention of delirium in hospitals is essential to avoid the discomfort and potential clinical complications of patients, as well as the suffering of their relatives, caregivers, and healthcare professionals. Policymakers and healthcare managers are encouraged to make the required efforts to prevent delirium in hospitalized older patients. (shrink)

Psychology research on men studies, attractiveness, and partner preferences has evolved from the influence of sociobiological perspectives to the role of interactions in shaping election toward sexual–affective relationships and desire toward different kinds of masculinities. However, there is a scientific gap in how language and communicative acts among women influence the kind of partner they feel attracted to and in the reproduction of relationship double standards, like the myth of the “warrior’s rest” where female attractiveness to “bad boys” is encouraged (...) or supported. Some women imitate “the warrior” behavior of men by choosing dominant traditional masculinities to have “fun” with and oppressed traditional masculinities for “rest” after the “fun” with DTM—choosing an OTM for a stable relationship, but perhaps without passion, while also feeling attraction toward DTM, a response which perpetuates the chauvinist double standard that the feminist movement has condemned when men behave in this sexist way. Through conducting a qualitative study with communicative daily life stories, this article explores, on the one hand, how language and social interaction among women can lead to the reproduction of the DTM role by women and, on the other hand, also how new alternative masculinities offer an alternative by explicitly rejecting, through the language of desire, to be the rest for the female warrior, the second fiddle to any woman. This has the potential to become a highly attractive alternative to DTM. Findings provide new knowledge through the analysis of communicative acts and masculinities evidencing the importance of language uses in the reproduction of the double standards in gender relations and to understand how and why these practices are maintained and which kind of language uses can contribute to preventing them. Implications for research and interventions on preventive socialization of gender violence are discussed. (shrink)

La crisis de representatividad de los partidos políticos de centro izquierda cuestiona la calidad de la democracia chilena y la proyección de los mismos. No obstante, constituyen referentes sociohistóricos con capacidad de adaptación a los cambios sociales y tendencias de época. Nos interesó conocer cual es el potencial de la generación de relevo de los partidos políticos de adaptarse a los cambios sociales y de representar a la nueva sociedad civil a partir del estudio de su condición generacional dentro de (...) campo político partidista. Los resultados indican que la condición intergeneracional, la visión heterocrítica frente a las formas de reproducción del poder político y la necesidad de legitimidad social favorecen la comprensión de formas distintas de hacer política que lo acercan a la nueva sociedad civil. Las prácticas políticas distintivas identificadas se asocian a la democracia de audiencia, la transversalidad de las alianzas y del territorio. (shrink)

The existence of shoulder abduction (SA) and finger extension (FE) movement capacity shortly after stroke onset is an important prognostic factor, indicating favorable functional outcome for the hemiparetic upper limb. Here we asked whether variation in lesion topography affects these two movements in a similar or a distinct way, and whether lesion impact is similar or distinct for left and right hemisphere damage. SA and FE movements were examined in 77 chronic post-stroke patients using relevant items of the Fugl-Meyer test. (...) Lesion effects were analyzed separately for left and right hemispheric damage (LHD, RHD) patient groups using voxel-based lesion-symptom mapping. In the LHD group, SA and FE were affected only by damage to the cortico-spinal tract in its passage through the corona radiata. In contrast, following RHD, these two movements were affected not only by cortico-spinal tract damage but also by damage to white matter association tracts, the putamen and the insular cortex. In both groups, voxel clusters have been found where damage affected SA and also FE, along with voxels where damage affected only one of the two movements. The capacity to execute SA and FE movements following stroke is affected significantly by damage to shared and distinct voxels in the cortico-spinal tract in LHD patients and by damage to shared and distinct voxels in a larger array of cortical and subcortical regions in RHD patients. No lograba situarme en el contexto que debía dar sentido a todo aquello. Después de muchas relecturas, creo haber conseguido cierta comprehensión, y me he dicho: quizás alguno haya pasado por experiencias semejantes y desee cotejarlas con las mías. La mía queda reflejada por entero en tres grandes perspectivas: 1) Crítica y razón a priori pura; 2) Estatuto (...) particular de la razón a priori pura; 3) Integridad de la misma. The impact of stroke on motor functioning is analyzed at different levels. ‘Impairment’ denotes the loss of basic characteristics of voluntary movement. ‘Activity limitation’ denotes the loss of normal capacity for independent execution of daily activities. Recovery from impairment is accomplished by ‘restitution’ and recovery from activity limitation is accomplished by the combined effect of ‘restitution’ and ‘compensation.’ We aimed to unravel the long-term effects of variation in lesion topography on motor impairment of the hemiparetic lower limb (HLL), and gait (...) capacity as a measure of related activity limitation. Gait was assessed by the 3 m walk test (3MWT) in 67 first-event chronic stroke patients, at their homes. Enduring impairment of the HLL was assessed by the Fugl–Meyer Lower Extremity (FMA-LE) test. The impact of variation in lesion topography on HLL impairment and on walking was analyzed separately for left and right hemispheric damage (LHD, RHD) by voxel-based lesion-symptom mapping (VLSM). In the LHD group, HLL impairment tended to be affected by damage to the posterior limb of the internal capsule (PLIC). Walking capacity tended to be affected by a larger array of structures: PLIC and corona radiata, external capsule and caudate nucleus. In the RHD group, both HLL impairment and walking capacity were sensitive to damage in a much larger number of brain voxels. HLL impairment was affected by damage to the corona radiata, superior longitudinal fasciculus and insula. Walking was affected by damage to the same areas, plus the internal and external capsules, putamen, thalamus and parts of the perisylvian cortex. In both groups, voxel clusters have been found where damage affected FMA-LE and also 3MWT, along with voxels where damage affected only one of the measures (mainly 3MWT). In stroke, enduring ‘activity limitation’ is affected by damage to a much larger array of brain structures and voxels within specific structures, compared to enduring ‘impairment.’ Differences between the effects of left and right hemisphere damage are likely to reflect variation in motor-network organization and post-stroke re-organization related to hemispheric dominance. Further studies with larger sample size are required for the validation of these results. (shrink)

Despite being fairly powerful, finite non-deterministic matrices are unable to characterize some logics of formal inconsistency, such as those found between mbCcl and Cila. In order to overcome this limitation, we propose here restricted non-deterministic matrices (in short, RNmatrices), which are non-deterministic algebras together with a subset of the set of valuations. This allows us to characterize not only mbCcl and Cila (which is equivalent, up to language, to da Costa's logic C_1) but the whole hierarchy of da Costa's calculi (...) C_n. This produces a novel decision procedure for these logics. Moreover, we show that the RNmatrix semantics proposed here induces naturally a labelled tableau system for each C_n, which constitutes another decision procedure for these logics. This new semantics allows us to conceive da Costa's hierarchy of C-systems as a family of (non deterministically) (n+2)-valued logics, where n is the number of "inconsistently true" truth-values and 2 is the number of "classical" or "consistent" truth-values, for every C_n. Vardanyan’s Theorems [36, 37] state that $\mathsf {QPL}(\mathsf {PA})$ —the quantified provability logic of Peano Arithmetic—is $\Pi ^0_2$ complete, and in particular that this already holds when the language is restricted to a single unary predicate. Moreover, Visser and de Jonge [38] generalized this result to conclude that it is impossible to computably axiomatize the quantified provability logic of a wide class of theories. However, the proof of this fact cannot be performed in a strictly positive signature. The system $\mathsf (...) {QRC_1}$ was previously introduced by the authors [1] as a candidate first-order provability logic. Here we generalize the previously available Kripke soundness and completeness proofs, obtaining constant domain completeness. Then we show that $\mathsf {QRC_1}$ is indeed complete with respect to arithmetical semantics. This is achieved via a Solovay-type construction applied to constant domain Kripke models. As corollaries, we see that $\mathsf {QRC_1}$ is the strictly positive fragment of $\mathsf {QGL}$ and a fragment of $\mathsf {QPL}(\mathsf {PA})$. (shrink)

Drawing from Goffman’s original observations on stigma and the consequences of interactions between the stigmatized and supportive or stigmatizing audiences, we conduct a 20-year review of the diverse literature on stigma to revisit the collective nature of stigmatization processes. We find that studies on stigma’s origins, responses, processes, and outcomes have diverged from Goffman’s relational view of stigma as they have overlooked important relational mechanisms explaining the processes of (de)stigmatization. We draw from those conclusions to justify the need to study (...) stigma as a collective phenomenon. We develop a relational perspective on stigma based on understanding how attributes are stigmatized (or not) by audiences in their interactions. We argue that to advance stigma research, it is necessary to build on Goffman’s theory to include the stigmatizers (i.e., the normal) and supporters (i.e., the wise); how they create, sustain, or remove stigma; and how they relate to the stigmatized (i.e., the targets). Accordingly, we provide a research agenda on stigma as a collective phenomenon that theorizes a relational perspective, proposes a typology of how audiences relate to stigmatization, and identifies patterns of relations between audiences. We thus offer a missing piece to existing accounts of stigma by focusing on the key role of audiences (i.e., stigmatizers or supporters of the stigmatized) rather than on the targets of stigma (i.e., the own). RESUMEN Largamente desatendida o malinterpretada, la noción de caos en la filosofía de Nietzsche es una pieza constitutiva de la particular concepción del ser que este autor habría dejado apenas esbozada. El artículo se propone elaborar este concepto en la obra nietzscheana, siguiendo algunas de las metáforas que lo iluminan. Desde allí se busca plantear los rasgos centrales de una ontologia del caos, de sesgo no metafísico, que, al afirmar el carácter acontecimental de la realidad, puede verse como precursora de (...) la ontología hermenéutica contemporánea. ABSTRACT The notion of chaos, which has long been neglected or misinterpreted, is a constitutive element of a peculiar conception of being that the philosopher only had time to outline. The objective of this article is to elaborate on this concept in Nietzschean philosophy, guided by some of the metaphors that shed light on it. This serves as the basis to set forth the main characteristics of a non-metaphysical ontology of chaos that asserts the eventful nature of reality and can thus be seen as a precursor of contemporary hermeneutic ontology. Este artículo1 explora dos obras cinematográficas guatemaltecas, Polvo (2012) y Nuestras madres (2019), dirigidas por Julio Hernández Cordón y César Díaz correspondientemente. En ambos largometrajes la denuncia sobre la violación de derechos humanos durante el pasado conflicto armado guatemalteco, la búsqueda de padres o familiares desaparecidos y sus secuelas en las víctimas constituyen la temática central. Por tanto, el análisis se centra en la importancia que tiene la búsqueda del padre desaparecido en los hijos, quienes, a raíz de esta vivencia (...) traumática, experimentan un vacío identitario, así como estragos individuales provocados por la falta de la figura paterna. Por otro lado, también se analizan el trauma y el silencio de las madres sobrevivientes como remanente del miedo/terror experimentado durante el conflicto armado, pero también como un mecanismo de protección para sus hijos. Fencing for the blind and visually impaired is an emerging sub-discipline of fencing that creates unusual conditions for meaning-making through interaction between embodied endowments and worldly affordances. With the rules of fencing slightly adjusted to the needs of the blindfolded participants – regardless of their sightedness – the discipline requires the fencers to engage in a duel by relying on other than visual cues. This article explores what an autoethnographic account of experiences of participation in fencing for the blind and (...) visually impaired brings to debates on the embodied, and specifically sensory difference. The discussion of these experiences intersects with debates on affect, affordance, and habit, with all three having important roles in related semiotic processes. Presented vignettes draw upon the author’s lived experiences of participation in fencing for the blind and visually impaired and are analyzed as part of a mixed-method autoethnographic study, accompanied by sensory methodologies, with a focus on an inquiry beyond the visual. The vignettes elucidate how we make sense of our surroundings through a complex engagement with the ecology of sensory and affective processes. In addition to exploring the role of affective and pre-conceptual aspects of our experiences, the article seeks to understand how semiosis occurs through both exposure to as well as the active pursuit of specific environmental signs available to us. The article also derives from biosemiotics to examine the complex relationship between meaning-making processes and habits. Finally, the autoethnographic account provides an insight into how we habituate the world and our embodied differences and thus enable meaning-making processes. (shrink)

This paper investigates the interpretation of counterfactual conditionals. The main goal of the paper is to provide an account of the semantic role of similarity in the evaluation of counterfactuals. The paper proposes an analysis according to which counterfactuals are treated as predications “ de re ” over past situations in the actual world. The relevant situations enter semantic composition via the interpretation of tense. Counterfactuals are treated as law-like conditionals with de re predication over particular facts. Similarity with respect (...) to particular facts is ensured by the semantics of tense in interaction with the modal, while the modal itself is responsible for invoking laws. In the paper, various arguments are provided to support a local view of similarity over the global approach found in semantics along the line of Lewis’s and Stalnaker’s. Arguments are also provided tying the evaluation of similarity to the interpretation of tense. Finally, arguments are provided to show that in key cases, the approaches make comparable predictions. Resumen: Este artículo tiene por objetivo establecer las bases para una comprensión disciplinaria y epistemológica de la geopolítica, considerando su ubicación como campo de estudio y el principal debate entre la perspectiva clásica y crítica de la misma en América del Sur. En este sentido, se efectúa un abordaje histórico sobre la evolución de la geopolítica, decantando en la manera en cómo el conocimiento geopolítico se ha dado en la región mencionada, al tiempo que se señalan las dificultades y propuestas (...) de investigación en el campo señalado. Se concluye que una discusión epistemológica de la geopolítica no solamente ayudará a lograr una mejor construcción y entendimiento del conocimiento geopolítico, desde un punto de vista holístico, sino que ayudará a destacar el rol de la academia en la necesidad de ser el eje articulador de dicha discusión.: This article aims to establish the basis for a disciplinary and epistemological understanding of geopolitics, considering its location as a field of study and the main debate between the classical and critical perspective in South America. In this sense, a historical approach is made on the evolution of geopolitics, decanting in the way in which geopolitical knowledge has been given in the region while pointing out the difficulties and research proposals in the field. It concludes that an epistemological discussion of geopolitics will not only help to achieve a better construction and understanding of geopolitical knowledge, from a holistic point of view, but also highlight the role of academics to articulate the discussion. Chalmers and Dennett are at opposite sides of the debate on the problem of conciousness. For Chalmres, conciousness is an unquestionable fact that cannot be explained by something else. For Dennett, what exists is really multiple judgements about our conciousness. Each author accuses the other of circularity. This is only possible because the difference between the two theories is actually a difference of principles. The same opposition that we find in their theoretical apparatus we also find on their more fundamental (...) and basic premises. This feature .makes it very difficult to choose one of the two theories, while it also radicalizes the difference bewteen them. On one side we have arguments that can refute intuitions, on the other we find that that one must first scrutinize our intuitions to then create arguments based on them. Between the two extreems we meet with the old dilemma of "what came first?". However, more important than to choose sides is to show how difficult the choice is.Chalmers e Dennett se encontram em lados opostos da discussão do problema da consciência. O Argumento dos Zumbis proposto por Chalmers, ao contrário de defender o dualismo, bane as qualia para um “mundo” onde elas não podem influenciar o julgamento que fazemos sobre nós mesmos. Por este motivo, pelo próprio argumento, podemos ser um zumbi e não saber. A isso Chalmers chamou de The Paradox of Phenomenal Judgment. O problema é que ele aceita tal paradoxo como parte de sua própria teoria. No entanto, este movimento filosófico não é aceitável e este paradoxo mina a (...) teoria de Chalmers por dentro mostrando que o argumento dos zumbis é, na verdade, um argumento contra o dualismo. Chalmers tenta resolver este problema com uma série de argumentos que tem como base o fato de que a consciência é um bruto explanandum indubitável. No entanto, tal tentativa fracassa por uma série de razões que mostram que mesmo se ele estivesse correto, ainda poderíamos ser um zumbi e não saber. (shrink)

Mary Livermore'sMy Story of the Waris a valuable piece of travel writing written from the point of view of a nurse who documented her unexpected personal and professional journey to administer the Sanitary Commission of the United States Union Army and provide nursing care during the American Civil War. Although Livermore's pre‐war background had not been solely limited to the domestic sphere, her wartime experience involved a public negotiation between the traditional domestic realm assigned to women and new nursing professional (...) functions that emerged during the war. In a context in which the general access of women to public writing was rather limited and in which nursing was not a formally regulated professional activity, Livermore's triumphal narrative reflects the increasing connection between progressively professional nursing functions that emerged in the context of war and a new women's rights leadership forged during her autobiographical journey. Cette étude vise à comprendre les spécificités sémantiques des lexèmes _manipuler_ et _modifier_ afin d’éclairer l’usage qui en est fait en bioéthique lorsque les actions désignées ont comme objet l’embryon, le cerveau, l’ADN, l’Homme, le vivant ou la procréation. En s’appuyant sur les définitions des dictionnaires et sur les réponses à un questionnaire soumis aux locuteurs, et en se servant de concepts de la sémantique combinatoire, de la sémantique argumentative, de la sémantique des cadres et de l’aspect sémantique, l’étude identifie (...) les éléments de signification propres à chacun des deux lexèmes. Ces éléments sont étroitement liés au potentiel de signification des lexèmes et aux traits aspectuels des procès qu’ils désignent. À l’issue de cette étude, un ensemble de traits sémantiques caractéristiques de chaque lexème est proposé, dans une perspective contrastive. (shrink)

Margaret Archer plays a leading role in the sociological analysis of the relation between structure and agency, and particularly in the study of reflexivity. The main aim of this article is to discuss her approach, focusing on the main contributions and limitations of Archer’s theory of reflexivity. It is argued that even though her research is a pioneering one, proposing an operationalization of the concept of reflexivity in view of its empirical implementation, it also minimizes crucial social factors and the (...) dimensions necessary for a more complex and multi-dimensional study of the concept, such as social origins, family socialization, processes of internalization of exteriority, the role of other structure–agency mediation mechanisms and the persistence of social reproduction.
